The counter shaft has both forward and reverse gears. Each forward gear has a corresponding gear on the output shaft and they are meshed permanently. The reverse gear also has a corresponding output shaft gear and an additional idler gear that slides between them when the reverse gear is engaged. This makes the reverse gear turn in the opposite direction to that of the forward gears. Depending on the kind of automobile, there may be one or two counter shafts.

A counter shaft may fail due to bad bearings. Symptoms of a faulty counter shaft include a hard shifting and reduced engine efficiency.
